Major Bupleurum Decoction  where the 'major" formula is given when  'minor" formula for treating the same symptom pattern has not been successful. (Minor Bupleurum Formula treats  FIRST Stage of Disease. . Sometimes patients taking Minor Bupleurum Formula for a few days then showed symptoms of the Second Stage of Disease, for which Major Bupleurum Formula is then given.

Like other formulas containing bupleurum, Major Bupleurum Formula treats "half-exterior, half-interior" diseases. When use of Minor Bupleurum Formula enables the person to return to the Second Stage of Disease, the disease-causing energy has weakened but not entirely succumbed to the action of the herbs. Sometimes "right Qi" prevails, and there is a feeling of heat radiating out of the body to sweat out" the disease. Sometimes "evil Qi" prevails, and there is a sensation of chilling cold. The symptoms of uncontrolled vomiting, despondency, and irritability will be more severe in patients who have returned to the Second Stage of Disease than in patients whose condition never progressed past the Second Stage of Disease.

The herbs in this formula are specifically helpful for disorders of the gallbladder and stomach. Vomiting treated by these herbs resulted from a reactivation of gallbladder energy overwhelming the stomach, rather than an inherent energy disorder in the stomach itself

Major Bupleurum Decoction has been analyzed in at least twenty-seven scientific studies. It reduces the rate at which glucose enters the bloodstream from digested food after meals and increases the efficiency of insulin.1 It is especially useful for treatment of persons who have diabetes with long-term gastrointestinal disorders such as gastroparesis. The formula also reduces concentration of cholesterol in the bile, inhibiting the formation of gallstones.
